Warranty Replacement
If this bulb does not operate for 3 years (based on 6 hours per day / 7 days per week of normal consumer use) from date of purchase when used as directed, your product is covered under our warranty. For returns, replacements or exchanges within 30 days of purchase, please return to the place of purchase.
To file a warranty claim, please fill out the below form. Once we receive the below form, we will confirm your purchase and warranty claim and email you with shipping instructions. You will be responsible for shipping the product with proof of purchase, register receipt and your name and address to Modvera Lighting. (Consumer Warranty), 8875 Whitney Drive, Lewis Center, Ohio 43035. Modvera will send you a replacement or at Modvera's option refund the original purchase price. REPLACEMENT OR REFUND IS MODVERA LIGHTING’S SOLE WARRANTY OBLIGATION. INCIDENTAL AND CONSEQUENTIAL DAMAGES ARE EXCLUDED.
